In practice that means custom functionality is written rather than assembled from plugins, updates do not routinely break customisations, and the admin is clean enough that staff use it without being retrained.
Yes, and it is a large share of the October CMS work we do. Content, URL structure and search equity all carry across. The redirects and the URL map are the part that determines whether a migration costs you traffic, so that is where the care goes.

Simple sites run four to eight weeks. Applications with real logic behind them run two to four months. Migrations depend almost entirely on how clean the content is in the system you are leaving, which is what discovery is for.
It is our default platform, we have been an official partner since 2019, and the Laravel foundation is the reason. Even in the worst case, what you own is a Laravel application with a content layer, which is a far more portable position than a proprietary platform or a heavily plugged-in CMS.
